Why Damp-Heat formulas get a long run

Damp is the slow one. In Chinese medicine it's heavy, sticky and very fond of staying put, and the old view is that once it's settled in the channels around the joints it won't pack up and leave in a fortnight. So formulas like this were taken over weeks, not days. At four tablets a day a single bottle lasts fifteen days, which means six bottles carry you through about three months. Handy, if your knees tend to grumble from October right through to spring, or the stiff, heavy feeling turns up every time the weather goes muggy.

Three months is also long enough to notice which way things lean. Herbalists watched for whether the picture stayed warm and puffy, which suits this formula, or turned cold and worn out, which points somewhere else. A notebook is more use than you'd think.

15 days a bottle, about 90 days in all

Ninety days, two bottles at a time

Bottles 1–2

Days 1 to 30

Two in the morning, two in the evening. Note how your joints feel first thing and after a wet day, since stiffness on waking is just the sort of thing herbalists asked about.

Bottles 3–4

Days 31 to 60

Keep going at four a day. Is the heavy, warm feeling still the main thing, or has it drifted towards cold and achy? That's useful to know.

Bottles 5–6

Days 61 to 90

Look back over three months. If things now feel cold and long-standing, herbalists would usually move to a warmer formula. Ongoing joint pain or swelling is one for your GP.

Pregnant, trying for a baby or breastfeeding? Then skip it, honestly. Cyathula, turmeric and Japanese knotweed are all Blood-movers, which is also why it's a no if you're on warfarin or another blood thinner. Any regular medication at all? Pop in and have a word with your GP or pharmacist first, we'd rather you did.

Choosing between them

Three formulas for stiff, unhappy joints

How did herbalists sort out creaky joints? Mostly by how they felt to the person. Warm and heavy got one formula, cold and weak another, and the fixed, sharp sort a third.

Guan Jie Yan Wan: your questions

Is Guan Jie Yan Wan used for arthritis?

Not as a treatment, but it often comes up in conversations about arthritis, because 'guan jie yan' is the everyday Chinese word for it, and the formula grew out of the old idea of Bi, or obstruction. So it's a name and a bit of history. That's it. It's a food supplement, and if a joint's sore, swollen, red or stiff and it's new or just won't settle, get your GP to have a look.

Does the weather really matter?

In Chinese medicine, very much. Damp is one of the outside influences, and wet, muggy weather is said to feed it, which is why so many people notice their joints when the season turns. Plenty of people start in early autumn and carry on through the wet months. A British winter gives the theory plenty to work with, frankly.

What did herbalists suggest eating alongside it?

Less of what the theory calls Damp-forming food: rich, greasy dishes, lots of dairy, sugar and beer. More light, plain cooking, and Job's tears turns up in Chinese soups for exactly this reason. Keeping moving mattered as well. Damp, the old herbalists would tell you, loves nothing more than a long afternoon on the sofa.

Who traditionally took it?

Mostly adults whose joints felt heavy, stiff and a bit warm, rather than cold and frail. Walkers, gardeners and people who spend their days on their feet tend to like it in their routine. It's less of a fit for someone elderly and worn out with cold, deep aches, where a warmer, more nourishing formula was the usual choice.

How long do six bottles last?

About ninety days. Guan Jie Yan Wan is a four-a-day formula, two in the morning and two in the evening, so each bottle of sixty lasts fifteen days. Six identical bottles of the same formula as the single comes to roughly three months, or one long, wet season. Keep the unopened ones sealed and cool.
