A long-standing ache, met with patience

Sun Simiao wasn't writing for a twinge after gardening. He had in mind someone whose back and knees had been cold and stiff for so long that it had worn down their deeper reserves. In the jargon that's long-standing Bi with Liver and Kidney deficiency. Years in the making. So a fortnight? Nobody was daft enough to expect much from that. Herbalists gave it a proper run, often right through the coldest months, and the test was dead ordinary stuff (how the knees were on a wet morning, say, or how you got on standing up out of your armchair).

It builds as well as clears, so it's a big one. Fourteen herbs! Rehmannia's in there, and a good whack of bitter gentian. If your stomach's on the fussy side, have it after food, honestly, and the weeks go a lot easier.

10 days a bottle, about 60 days in all

Sixty days through the damp season

Bottles 1–2

Days 1 to 20

Three after breakfast, three after tea. And wrap up! What's the use warming you from the inside, herbalists would say, if you're stood out in the drizzle?

Bottles 3–4

Days 21 to 40

How are the back and knees taking the weather? Cold mornings especially. A gentle potter about every day was always part of it, too.

Bottles 5–6

Days 41 to 60

Have a think back. Joint gone hot, red, swollen? Back worse, or pain running down a leg? Then it's your GP you want, not more tablets.

Pregnant? Then Du Huo Ji Sheng Tang's not for you, sorry, as achyranthes has always been kept away from pregnancy. Breastfeeding, ask your GP. On warfarin? The dong quai and lovage won't get on with it, and liquorice can push blood pressure up. Anything you take regularly, have a word with your GP or pharmacist first.

Choosing between them

Cold, hot or simply weak?

Aura has a few formulas herbalists used for stiff backs and knees. The old question was whether the ache felt cold, hot or just worn out.

This page

Du Huo Ji Sheng Tang

Cold, damp, stiff back and knees, in someone who's run down underneath it all. Rain makes it worse; a hot water bottle, better. Clears and builds in one go.

When joints feel warm

Guan Jie Yan Wan

The opposite temperature. Picked when Wind, Damp and Heat seemed lodged in the joints, so they felt heavy, stiff and a bit warm to the touch.

Inner fire burning low

Jin Gui Shen Qi Wan

Kidney Yang deficiency. For the one who's always freezing, back, knees and feet, and who's slowing down all over. Less achy joints, more pilot light gone low.

Du Huo Ji Sheng Tang: your questions

My knees creak. Is Du Huo Ji Sheng Tang the arthritis one?

Its name does pop up whenever osteoarthritis gets mentioned, mostly among older folk whose knees and backs can tell you it's going to rain. Historically that's just the kind of person herbalists gave it to. It isn't a treatment for arthritis, though. Persistent joint pain deserves a GP's assessment, and a joint that's suddenly hot, red and swollen needs seeing the same day.

Is winter the best time to take it?

Many herbalists thought so. Cold and damp were the outside influences Sun Simiao wrote about, and British winters supply plenty of both, so it's often brought out once the clocks go back. Some people who feel the change in the weather also start it in autumn, before the worst of it. You can take it any time, mind. Winter's just its home turf.

And how long would a herbalist have kept you on Du Huo Ji Sheng Tang?

Weeks. Months, often. Well, the ache had been there for years, hadn't it? A practitioner would keep popping back to tweak things, maybe winding it down once spring warmed up. Nobody thought of it as a quick fix. A couple of months was a pretty normal first go, and then you'd decide.

Could I have it with Yao Tong Pian as well?

We wouldn't double up, not unless a practitioner tells you to. They're both warming lower-back formulas, both have dong quai, so there's a lot of overlap. Herbalists picked one. This for cold, damp stiffness; Yao Tong Pian when the back felt weak and tired more than cold. Which sounds like you?

Sixty days from a 6-pack, is that right?

Yep, sixty. Six 60-tablet bottles of the same fourteen herbs as the single, three tablets morning and evening, ten days a bottle. Call it two months. For most people that's first proper frost to, oh, somewhere near the back end of winter.
